Enclose self-attested photo copies of the following documents / testimonials along with the application form:

• Valid Driving License (cab) issued by the Competent Authority
• PAN Card issued by the Income Tax Authority or Aadhaar Card issued by UIDAI or Voter ID Card issued by the Electoral Registration Officer.
• HSLC Certificate, Marks Sheet and Admit Card
• Certificate of Caste from Competent Authority in respect of candidates belonging to ST(P)/ST(H)/SC/OBC/MOBC wherever applicable.
• Up to date Employment Registration Certificate issued in the State of Assam.

Note: One copy of recent coloured photograph should be attached on the application form and another 3 (three)copies of the same photograph should be submitted separately along with the documents.
